I prefer botf for qot, staff of renewal for Da, and swift anklet for just about everyone else. Seeing how I onl play qot, Da, and TB .

Rook - Blood or Renewal

Demon Assassin -EoM always

Beast - Blade of the Serpent always, even for ooze, thanks to Magla for the schooling... you give up something early, but late game without the need for a helm you are rediculously ferocious

Reg - It doesn't matter much, but BotS usually, maybe anklet if there is no EB or Oak (rarely seen) 

Sedna - Blood, but I am bad with Sedna so what do I know.

QoT - Blood, anklet, cloak of night or warmage depending on build and strat (they are all bad, no need to point it out)

Oak - Blood all the way

Erebus - Blood or HoB

Torch - Swift Anklet always

I'd say BotS on fire TB and Swift on Ice... and on hybrids either one.

Rook - Blood

Demon Assassin - Blood

Beast - Blood

Reg - Blood

Sedna - Blood

QoT - Blood

Oak - Blood

Erebus - Blood

Torch - Blood

I wish I could get renewal for QoT. OMG Pacov Hax.

Ah you kids... I haven't ever chosen anklet in high level games on erb.  I have in pantheon matchups once I realize I'm playing weaker players.  Just having fun.  Lolz... biggest mistake EVER... there's the bay of pigs invasion, war in vietnam, and pacov choosing swift anket on erebus. 

I do stand by amulet of TP on rook.  I find it quite valuable as my rook is typically all over the map as needed once i hit 5, especially on cat. 

Re: QoT - yeah, I hax.  I'll edit the post to remove that... kinda drugged up on flu medicine today.

bots + rain of ice means you pretty much get your mana back immediately.  Give it a shot.

You still get mana from BotS for it. 500 mana in, you get 5 * 200 * .8 == 800 mana out. And that's just level 1. You still get mana from "overkill"  so even if all you kill are minotaurs with your 800 damage Rain of Ice IV you still get 7 * 800 * .8 ~ 4500 mana for a 1100 mana investment

Fireball lvl 2 and up nets you positive mana.

I'd sell my soul to the Blade of the Serpent if possible.

I'm going to throw an idea out there. What do you guys think of oak with wings of the seraphim? 

+200 hps aura for 10 seconds, damage breaks the effect. Sort of a poor man's heart of life, and in the early game, it is practically the same thing. You could throw on a shield, activate the wings, and gain 400-2000 health mid battle.

Also, what about taking a second look at the magical coin purse. You could start out with say, banded armor, scalemail, AND a potion, tp, or capture lock. It's something to think about.

Would you sell your priests and shamblers for it as well?

I think coin purse is closer to being very good, but I think the sweet spot would be 25%.  Not sure if that would make it op, but I'd certainly consider giving it a shot.
